En 1985, Ivan Drago, un boxeur originaire d'URSS, débarque aux États-Unis pour une série de matchs d'exhibition. Lors d'une conférence de presse, il affirme n'avoir aucun intérêt à boxer contre Apollo Creed, le grand ami de Rocky, car il le battrait facilement. Vexé, Apollo le défie officiellement, malgré les réticences de Rocky. Il est tué sur le ring et Rocky va demander un combat contre Ivan Drago pour se venger. Cette confrontation se déroulera sur le sol russe.
